Ok, I got it to work now.  I had upgraded to a recent version of bttv, but I did not notice that doing a 'make install' did not copy over bttv.o so I was still using the older version.  The newer versions bttv-0.7.102/103 work just fine and I moved the drivers with my Linux sources.

card=52 is wrong and would explain silence.

So
- Don't use any "card=" in modules.conf
- Directly connect active speakers (or headphone) to Line Out
- optionally: upgrade to bttv-0.7.103 (then even tuner will be autodetected)
