The sticker has nothing underneath, or any identifying marks at all, just a Jetway sticker.
A picture fot eh card can be seen at

Its not a very good picture, but I have no camera to take one myself.

The main chip is Conexant Fusion BT878A.
I can provide any other information about the card, i.e. chip numbers.
