Try peeling that label off to see the manufacturers label.  That should 
have the info you need to select the right tuner.  I had to do this for 
my Zoltrix Genie/TV.

Sounds like you have the wrong tuner selected.  Look at the label on 
the tuner and choose the closest match from the CARDLIST file 
(/usr/src/linux/Documentation/???).  Then add the following lines to 
/etc/modules.conf:
options tuner type=<From CARDLIST>
options bttv card=<From CARDLIST> tuner=<from CARDLIST> radio=<0 or 1 
if the card supports FM>

Unload and reload bttv and tuner and see if it works.
