Ok, an update on this,
Shortly after I wrote this message (2 hours or so) the video blacked out again. I decided to investigate the advice posted recently and look at the slot number, irqs, etc. I noticed the card was sharing an IRQ with the AGP slot (NVIDIA GeForce2 card). I've swapped the card's slot, and setup the bios so that the FlyVideo 3000 is now the only device on its IRQ.
So far 2 days and going strong :)

This was running on 0.1.10, I've just upgraded to 0.1.11, powered off and rebooted and its working again. I've turned every debugging parameter I could find on, so we'll give it another 4 days and I'll let you know how it goes...
