Yes I have the same problem. I can get the video restored sometimes
if I change channels or toggle fullscreen. Otherwise, I may need to
re-insmod the module, or reboot (as you observed).

My guess is that if the video is rather dark/dim, it triggers this
problem. I don't know what the cause is though.

I think that has to do with the muting on your speaker out. Try using
aumix to mute your speakers or something?
