No.

Yes.  It fixes a number of v4l(1) API flaws.  It provides timestamps for
the captured frames, has ioctl's to to list the available formats, ...

No.  v4l2 provides backward compatibility to v4l1, so all v4l2 drivers
can be used with the v4l1 API too.

Don't know.  There are a number of v4l1 usb cam drivers in the
standard kernel.
