I've followed Barrie's HOWTO and unfortunately still no go. 

To make sure there is no mistake with the wiring or mixer settings for
my sound card I have the audio out of the card going directly to an amp
and speakers. 

A couple of interesting points to note: 

I can tell when the card is initialised as there is a small pop from the
speakers. 

It doesn't seem to matter if the audio out from the card is originally
from the onboard tuner or is being input to be captured. 

If no mixer is specified it selects mixer2 by default (in my case
anyway) - however I don't have a /dev/mixer2. Is this what its referring
to?  (I have tried mixer1 - which I do have but makes no difference)

Any ideas people? 

The kernel log is as follows, Everything is pretty much the same as
Barrie's except the IRQ sharing.
