Take a look at file /etc/security/console.perms
Comment out all lines containing "video" or "v4l".
It should work better.
(I didn't understand clearly the role of this file, but everytime a
"normal" user logs on, it modifies the permissions on the files listed in
console.perms).

chown root:sys /dev/video0
chmod 777 /dev/video0
while sys is a group that I added apache to
and apache is the user under which (per linuxconf) apache accesses CGIs
I got that script linked in rc3.d and rc5.d
sometimes I get that failure
even when the permissions were still fine
Sometimes those permissions don't seem to get changed or get changed after
the change - What would be doing that ?
Sure there is something I should do better - Am all ear !

and one with an avermedia BT878 based card
