bttv-0.7.54 + TEMIC tuner patch

Hi,
this patch:

- adds 5 more TEMIC tuners (potentially found on Hauppauge cards)
  and implements hauppauge_tuner_id -> bttv_tuner_id mapping

- protects against further confusion by adding TEMIC model numbers 
  (e.g they have at least 3 different PAL_I models in the 
   series 40x2, 40x6, 40x9)

- fixes wrong tuner detection with one Hauppauge PAL_I model (see above)

- add some comments about magic values to make it more understandable
  without having to read bunchs of datasheets first

- handle 4046FM5 as PAL. This can receive multiple standards, but we must
  switch it by i2c commands (API to be developed, probably wait until
  standards switching chips become more common).

OPEN ISSUE:
This line from bttv-0.7.54 seems wrong, because it takes wrong IF for NTSC:
 #define TUNER_TEMIC_NTSC_M      17
Can somebody explain this (please see my comments in the patch)?
Please remove tuner=17, Gerd, if you agree. Please apply if you like.
