i have here exactly the same problem :-(
I have a Flyvideo 3000 bought in germany and my sound only works every x 
boot. Sometimes I have no problems but sometimes it doesn't work.

i make 
modprobe saa7134 card=2
v4l-conf
motv

sometimes it works but sometimes I only get the picture but no sound.

if I now make

modprobe -r saa7134
modprobe saa7134 card=3

I can hear the sound from the channel I just watched without starting motv.
If I start now motv I see only white noise and hear also white noise :-(

When I load my card with card=2 I can see the pictures but motv shows that 
is only working in mono mode but the channels are definately stereo. 
Sometimes my card works perfectly and then it shows me stereo.

If sound isn't working the saa7134[0] mixer (I use kmix) only shows two 
channels, if sound works, the mixer shows three channels.
