I do also have those files, but my videodev.h is much smaller than yours.

Did you apply another patch (videodevX for example?). Maybe you should start 
from scratch. Unpack a fresh kernel-archive 2.4.18, apply patch 2.4.19-rc3, 
apply patch v4l2-01-add-api. Fix ksyms.c, and try to compile
